The Canadian Association
of Independent Life Brokerage Agencies ("CAILBA" "we",
"us" "our") is an organization whose members are life
insurance industry participants, a majority of which are independently
owned life and health insurance brokerage agencies. If we collect, use
or disclose personal information identifiable to individuals we do so
in accordance with the principles set out in this Privacy Statement, which
may be amended from time to time.
Personal
Information About Members
In the course of carrying
on its activities on behalf of its members, CAILBA collects very little
personal information about members or individuals who are representatives
of member companies. For the most part the information that is collected
consists of the name, title and contact information of an individual in
their capacity as an employee or representative of a member company.
If we do collect,
use or disclose personal information about a member or an individual who
represents a member, we are guided by the general privacy principles set
out in the next section.
From time to time,
we do offer seminars and accept registration over the Internet, over the
phone, by email or mail communication. In that case, we will collect only
such information as is required to ensure (a) that a confirmation is sent
to the address or e-mail address of the registrant's choosing and (b)
that payment is processed correctly. We will ask for a registrant's name,
mailing address, phone number, e-mail address and credit card information.
Credit card information provided on-line shall be processed in an environment
with appropriate security in effect. If a member or a representative of
a member provides us with his or her contact information, including his
or her e-mail address, we will communicate with such individual about
CAILBA events and activities using that information. We rely on members
or representatives of members to keep us up to date on their particulars.
General
Privacy Principles
- We only collect that personal
information which is necessary in connection with a CAILBA event or
activity.
- An explanation will be provided
at or before personal information is collected as to what personal
information is being collected and how it will be used.
- We do not collect, use or
disclose personal information about an individual without his or her
knowledge.
- Except where required or permitted
by law, we do not collect, use or disclose personal information without
an individual's consent.
- We do not sell or rent our
membership list.
Currency
of Member Data Collected
CAILBA may contact a
member or individual representing a member requesting updates of data.
This will normally be done by regular mail or by an e-mail directing the
recipient to the appropriate website. We do not use third parties (other
than LOGiQ3 Inc., our manager) to collect this data on our behalf. If
you receive any communication from anyone purporting to perform this function
on our behalf or if the authenticity of any communication is doubtful,
please contact CAILBA immediately.
CAILBA
Debt Monitoring System
We operate a debt monitoring
system (the "DMS"), on behalf of both insurance companies and
managing general agencies which are CAILBA members. The purpose of the
DMS is to ensure the availability and accuracy of facts about the existence
of outstanding and overdue debts owed by insurance intermediaries to CAILBA
members. Members will obtain information through the DMS in connection
with an application to become authorized to sell insurance on a member's
behalf and members will input information to the DMS in the event that
an insurance intermediary does not pay amounts owing in accordance with
its contractual obligations to such member. A request for additional information
or a request to review or correct one's personal information should be

Privacy Matters
When a user visits CAILBA's
website, some general information about the visit may be recorded,
such as:
- The Internet domain for the
user's Internet service provider, such as "company.com"
or "service.ca" and the IP address of the computer accessing
the website, such as "ppp-55" or "123.123.123.123";
- The type of browser (such
as "Netscape version x" or "Internet Explorer version
x") the user is using;
- The type of operating system
the user is using (such as Macintosh, Unix, or Windows);
- The date and time of the visit
to our site and the web pages that were visited on our site, along
with the address of the previous website the user visited, if the
user linked to our site from another website.
We use this information
for statistical analysis and to help us make our site more useful to visitors.
We may disclose this information to third parties such as sponsors or
clients and/or use it for auditing purposes.
We may use "cookies"
that identify a user as a return visitor. A cookie is a piece of data
that a website can send to a user's browser, which may then store the
cookie on the user's hard drive. So, when a user returns to our site again,
we can tailor information to suit the user's individual preferences. The
goal is to save time and provide a more meaningful visit and to measure
website activity. Browsers such as Netscape allow cookies to be disabled
or the will inform the user when a cookie is being stored on his or her
hard drive.
